Student and Parent Awareness Form

A paper entitled “6th, 7th, and 8th Grade Band 2008-09” was sent home with each participating band student to describe the purpose and policies surrounding the Middle School Band Classes at Mardela.  The following list of main points are presented to student and  parent to read and check-off to confirm that the information was received and understood.  Together, we will build great musicians.

I will make sure that a pencil, book and instrument are present at every class period.

 

 

I understand that regular practice (2 hours per week) at home is 20% of the final grade.  A practice calendar must be kept and signed by a parent, then returned to Mr. Boltz the first of each week.

 

 

I have transferred the performance schedule to my calendar.

 

 

I understand that there will be mandatory, graded practices happening after school, 3-4:15p, once a week October – December Concert, February/March to Spring Concert.

 

 

I understand that any absence from after school practice should be followed by a note of explanation for it to possibly not count negatively.

 

 

I understand that 3 missed after school practices may remove a player from the upcoming performance, with grade reduction.

 

 
 

***SPECIAL NOTICE***

The middle school uniform features a polo-type shirt (borrowed by the student at no cost).  The uniform will be completed with dark black pants, black socks, and black shoes (provided by the student, approved by Mr. Boltz).  At the end of the school year, the polo shirt will be washed, folded and returned to the band; those not returned will be paid-for by the student at replacement cost.
